Specialty coding

Specialty and E/M codes, in one place.

The codes each specialty we bill uses most, plus the office visit levels and modifiers behind them. Built by our coding team for practices, front offices and new billers.

Outpatient practice coding only; we do not bill hospital or facility claims. Reference only: code from the documentation, and verify against the current year's CPT and ICD-10-CM code sets and your payers' policies. Descriptions are plain-language summaries, not official code descriptors.

Generic E/M codes

Office and outpatient visits.

Since 2021, office visit levels are chosen by medical decision making (MDM) or by total time spent on the date of the encounter, whichever supports the higher level.

CodeMedical decision making
99202Straightforward15+ min
99203Low30+ min
99204Moderate45+ min
99205High60+ min

Time thresholds are total minutes on the date of service and must be met or exceeded.
